Every incident gets logged here — injuries, near misses, equipment failures, animal escapes, weather damage. If it happened and it matters, write it down.
How to log an incident
- What happened — plain language, no jargon
- When — date and time
- Where — which area of the site
- Who was involved — staff, animals, visitors
- What was done — immediate response
- What needs to change — prevention for next time
